The El Centro City Council unanimously approved the transfer of El Centro Regional Medical Center (ECRMC), including its properties and related assets, to the Imperial Valley Healthcare District (IVHD) during a special meeting on Oct. 13, advancing a transition that has been under negotiation for three years.

The transfer is expected to be finalized Thursday, Oct. 15, marking the end of the hospital’s ownership by the city of El Centro and placing the facility under the governance of a regional health care district.

The transition follows the passage of Assembly Bill 918 in October 2023, which established the Imperial Valley Healthcare District as part of an effort to consolidate health care governance and develop a unified system serving Imperial Valley residents.

Since the legislation was enacted, city officials, hospital administrators and district representatives have worked to address financial obligations, legal requirements and operational considerations associated with transferring the medical center.

“This is a significant moment in the history of El Centro and healthcare throughout the Imperial Valley,” Mayor Michael Crankshaw said in a statement.

“For three years, this process has required extensive collaboration, difficult decisions, and a shared commitment to the future of healthcare in our region. Today’s unanimous decision brings us one step closer to completing this important transition,” Crankshaw said.

The City Council initially approved the Asset Transfer Agreement in July 2025, and the agreement was formally executed Aug. 1, 2025.

Since then, representatives from the city, ECRMC and IVHD have continued working to satisfy the conditions outlined in the agreement and prepare the remaining documentation required to complete the transaction.

Under the agreement, the Imperial Valley Healthcare District will assume certain financial obligations associated with the hospital, including responsibilities related to existing financing arrangements.

The city’s announcement did not specify the total value of the assets being transferred or the amount of financial obligations the district will assume.

The transfer represents a change in the hospital’s ownership and governance structure, shifting responsibility from the municipal government to the regional district established under state law.

ECRMC has served residents of El Centro and surrounding Imperial Valley communities for decades, providing medical services to patients throughout the region.

City of El Centro representatives acknowledged the work of hospital employees, medical professionals, and administrators who have continued providing health care services during the negotiations and transition process.
